Introduction
Pikeman Family Learning Centre is committed to providing a safe, nurturing, and high-quality environment for all children and families. In accordance with the Duty of Candour regulations, we recognise our responsibility to act openly and transparently when unexpected or unintended incidents occur that result in harm. Where such events arise, we will ensure that those affected are informed promptly, treated with respect and compassion, and offered a clear explanation and apology. We also use any incidents as opportunities to review and strengthen our practice.
Duty of Candour Overview
The Duty of Candour is a legal requirement which promotes openness, honesty, and accountability within care services. If an incident occurs that results in harm or has the potential to cause harm, the service will ensure that parents or carers are informed as soon as possible, provided with an explanation of the circumstances, and offered appropriate support. The service will also review the incident and implement any necessary improvements to reduce the risk of recurrence.
Reporting Procedure
All staff members have a responsibility to report any incident or concern that may meet the Duty of Candour criteria. Such incidents must be reported immediately to the Head of Centre. The Head of Centre is responsible for reviewing the incident, ensuring that appropriate communication takes place with parents or carers, and confirming that all Duty of Candour procedures are followed. Where required, the Head of Centre will notify the Care Inspectorate in line with regulatory requirements. A clear and accurate written record of the incident and any actions taken will be maintained.
